Eligibility

The children the Light House works with:

The service works with children who have a substantial, permanent and enduring physical disability or impairment and/or a moderate to severe learning disability. It is a service for profoundly disabled children with complex needs between birth and 17 years old.

Overnight breaks will be provided to children eight years and over (five years in exceptional circumstances) across Derby City and South Derbyshire that have severe challenging behaviour or who are profound and multiple learning disabled.

Children with severe learning disability who have social care needs living in Derby may also receive a residential overnight service.

How to access the service

To access the service a common assessment must be undertaken and accompanied with a signed consent form for the Light House.

This must be completed by a professional already working with the child, that knows the child and the perceived difficulties, eg school teacher, community nurse, health visitor, paediatrician, OT or physiotherapist that will identify any unmet needs.

Please ensure you complete all sections, state who the lead professional is, how you will review the child's needs and have the Light House consent form signed by a parent or carer.
