How we can support
As corporate parents, it is our responsibility to provide you with the best possible support and care.
Providing the best support and care means working alongside you to keep you safe and supporting you to meet your individual needs. We can help you to access housing, education and training, work experience, health services and come alongside you through your successes and times of difficulty. We will give you support, advice and encouragement to help you make the most of your journey as a care experienced young person.
Your personal advisor (PA)
The Leaving Care Service is based at the Council House, Corporation Street, Derby, DE1 2FS. Our team is made up of personal advisors (PAs) who have a duty to provide you with support and assistance. We provide this support to you up until your 25th birthday. The relationship between you and your personal advisor is an important one, so we encourage you to make the most of this relationship in order that you feel supported.
When you turn 18, you will no longer have an allocated children’s social worker, and your PA will form an important part of your network and will continue working with you at least until the age of 21.
Your PA will stay connected and arrange to meet with you regularly, they will visit you at home and in the community.
Your decisions and choices
As a young adult, the decisions and choices you make are your own – after all, you will have to live with the consequences. Your personal advisor will provide you with advice, information and guidance to help you make the best choices and decisions. The relationship between you and your personal advisor is important, make the most of your personal advisor, treat them with respect and keep in touch.
Your pathway plan
Young people between the ages of 16 to 21 who are looked after by Derby City Council or young people who are care experienced will have a pathway plan. In the months approaching your 21st birthday, your PA will meet with you to look at what your support will look like post 21. It may be that you no longer require a pathway plan, and you can access support from our service through regular drop ins and advice line. Additionally, it may be best for you to continue to have your pathway plan reviewed by an allocated PA.
Your pathway plan will be completed by your allocated children’s social worker if you are under 18 and then your PA once you reach adulthood. This will be done with you, and we encourage your participation in completing this. Your pathway plan will have information on the best way to support you towards and onwards into adulthood. It will cover accommodation, education, health, support networks, independence skills, finances and identity. Your pathway plan is written to you in a way that should meet your individual needs. It will capture your current circumstances, goals you wish to achieve and include your views and opinions.
Your pathway plan will be regularly reviewed with you, at least every 6 months. This is to ensure that we are providing you with the relevant support and advice to allow you to achieve your goals.
